Health Plan Training

The Annual Special Needs Plan (SNP) Model of Care (MOC) Training is a requirement by the Centers for Medicare and Medicaid Services (CMS) for all Medicare Advantage Special Needs Plan providers, as well as employees, temporary employees, contractors, consultants, and interns that have a departmental SNP responsibility.

Alignment Healthcare Additional Training

Actions Required

Delegate Representative will need to ensure all providers caring for SNP members receive this training deck for review and attest with signature of completion (review of the training deck with providers can be via email, live webinar, conference call, etc.) and provide a roster of providers that received the training.

Delegate Representative and providers to review training documents. Designated Representative to sign attestation on behalf of each applicable delegated IPA/MG. Providers are to sign the roster upon training completion. Attestation(s) and rosters are due within 30 days of receiving the SNP MOC Mandatory Provider Training Deck and Attestation from Alignment (refer to pages 23 and 24 of the training deck).

Upon completion of signed attestation and collection of provider signature listing(s)/roster(s), return to the AHP Quality Management Department via email to QI@ahcusa.com or fax to 562-207-4617.
